• Graduate Application Form: Submit one of the following application forms: 
  • Application Form for Degree Applicants
  • Application Form for Non-degree US Applicants
  • For online application, please visit our Application Center (Note: Please, do not submit paper-based application form if you are applying online).
• Statement of Purpose: Submit one copy of this document. Do not fill this out if you are electronically submitting. 
  • Statement of Purpose for U.S. Citizens or Permanent Residents | online form is here
  • Statement of Purpose for International Applicants | online form is here
• Letters of Recommendation: There is no form; (except for the applicants in Political Science); instead, please see instructions for preparing letter of recommendation.
• Application Fee Payment: If you intend to mail your application fee to the Graduate School, you must submit the Payment form along with the application fee (Note: Application fee is waived for expedited applicants and McNair scholars)
• Request Forms by Mail: If any or all of the above forms are not possible for you to download, then email usgradapply@aalan.ua.edu (if U.S. applicant) or intergradapply@aalan.ua.edu (if international applicant) with your mailing address to receive an application packet.

Financial Forms (for international applicants only)

• Certificate of Finances: Submit one copy of this form to show that you have adequate financial support to attend. Read guidelines before completing this form.

Immigration Related Forms (for international applicants only)

Health Center Forms

• The University Health Service requires all entering students to have a medical clearance. At the time of registration, you must present proof of a measles, mumps, and rubella (MMR) vaccination. Click here for a list of medical forms.
