Effective June 10, 2020, the forms listed below are available for digital completion and signing. The few forms which have not been converted will remain here as fillable PDF versions for a very short time – please continue to process these manually.. 

Processing these forms for digital signing will be administered through your academic department. UA Employees only may access these forms under the Employee tab in myBama.  Please contact your academic advisor/graduate director  in your department for submission of your form(s).

Master’s Students

Educational Specialist Students
